Tax Return Checklist


Here is a general individual tax return checklist to help you organize your tax information.  As each tax return is different, all of these may not apply to you.  In addition, more complex returns may require additional information.  This list is not comprehensive.  Any additional information necessary will be gathered during your appointment.  There is a comprehensive organizer checklist available for download at the bottom of this screen if needed.

Personal Information

  • Changes in dependents, Address, Marital Status
  • Social security #’s and birthdates for new members of the family
  • Medical insurance form 1095 (if ‘Obama care insurance’)

Income

  • W-2’s, 1099’s, Investment 1099’s (stock sales, dividends, etc) 
  • Crypto Currencies: Summary statements (excell/8949/sch D) showing crypto sales and cost basis, & possible cryto “earns”.
  • Business income/expenses, Rental income/expenses
  • Social Security benefits, Alimony received (executed prior to 1/1/19), Sale of property-escrow statement
  • (NEW for 2022) 1099-K if you had $600 or more in goods or services transactions on Venmo, PayPal, Amazon, Square, Ebay, Etsy or similar.

Adjustments to Income

  • IRA/Pension contributions, HSA contributions, Educator Expenses, Student loan interest (1098-E), Alimony paid (executed prior to 1/1/19)

Itemized Tax Deductions / Tax Credits

  • Child care costs, College costs (1098-T), Residential energy upgrades, Electric Car purchase
  • Mortgage Interest paid- form 1098,  Property tax, Medical/Dental expenses, Charitable donations, Auto licensing

Taxes You Paid

  • Quarterly estimated tax payments,  Last year’s income tax paid
